Prescribe and Pray is a deeply disturbing and highly realistic simulation of what it must be like to run a rural medical clinic in a doubtless forgotten community where faith healing is the thing; where one’s medical knowledge is almost constantly at war with the superstition and general shortage of supply. The gameplay consists of a fairly harsh turn-based cycle of diagnosis, treatment, and the management of resources: every day, patients coming with new symptoms reaching your ears through their own superstition are being described to you by the local healers, claiming a “curse of the stomach” for an ulcer or a “demon in the blood” for infection, you are then forced to interpret their fears, perform rudimentary exams with limited tools, and decide whether to prescribe actual medicine (which depletes your scarce stocks) or offer placebo rituals to maintain their trust and community standing.

The main tension comes from the “Faith vs. Science” meter; every genuine cure strengthens your medical reputation but may alienate the community’s spiritual leader, while successful placebo treatments deepen their reliance on prayer but risk real deterioration if you misdiagnose a critical condition. You survive by the practice of your knowledge, the faith you have, and your careful management of the resources. Every moment counts; you keep the antibiotics coming, make herbal remedies from plants you pick, and even barter with merchants for the things you can’t make.

At the same time, random events buffing or destroying your operation can be very faith-healing episodes, epidemics, or government inspections. The measure of progression in this is the number of lives saved, who have converted their trust to scientific and, the slow, quiet, and grinding triumph of keeping a community alive when both the heavens and the supply chain have left them.
